This appendix documents the condensed survey structure used to examine students’ perceptions of AI, autonomy, bias, fairness, trust, governance, societal impact, regulation, and AI ethics education. It provides a structured overview of the survey variables, response options, and thematic question blocks, including background and demographics, AI and autonomy, AI bias and fairness, trust and governance, societal impact and regulation, AI ethics learning, and open-ended reflections.

The appendix also specifies the analytic use of the survey items. Closed-ended items, including single-select, multi-select, and ordinal response formats, were used for descriptive context and orientation profiling. Open-text responses, particularly examples of personal AI use, future concerns or hopes, and substantive “Other” responses, were used for qualitative analysis. The instrument map included in the appendix links survey blocks, catalogue codes, item formats, number of items, and their role in the analysis.
